1. This offer is made by Revitive (Actegy Limited) (Registered number: 04819502), The Lightbox Willoughby Road Bracknell, Berkshire, RG12 8FB.

2. This promotion is available for a limited time from 25th March 2026 – 19th May 2026 inclusive on the Revitive Medic, Medic Knee and Medic Coach devices (excludes ProHealth) purchased from Boots either instore or online. This offer is not valid for purchases made from any other Retailer.

3. Free returns will only be available for shipments posted from mainland England, Wales and Scotland.

4. This offer is open to individuals who have purchased an eligible Revitive product and used it for a minimum of 28 days. If after trying the product, you are not satisfied with it, you may apply for a refund in accordance with these terms and conditions.

5. The guarantee period runs from the date of purchase.

6. If you are not satisfied with the product within the guarantee period, please:

a. Submit the refund request within 90 days from date of purchase by completing the online form (www.myrevitive.com) or by contacting our Consumer Care Team on 0800 014 6377; and

b. Providing the original proof of purchase - receipt or invoice (photocopies are not accepted);

7. Only one refund request will be accepted per household.

8. The product must be received by us within the guarantee period.

9. Money back will be paid by bank transfer (BACS), for the price paid (net of any promotion or discount), within a period of 28 days of receipt of the product and completed documentation.

10. No responsibility can be accepted for claims lost or delayed in the post, or incomplete or illegible forms. Proof of posting will not be accepted as proof of delivery.

11. The device should be returned in full working condition, showing only normal wear, in its original packaging and with all accessories and manuals included.

12. All care should be taken to ensure the returned product is adequately packaged and protected from transit damage to avoid loss or damage. We reserve the right to deduct from the refund the cost of rectifying any loss or damage to the product.

13. We will not accept any liability for additional costs associated with the return, proof of delivery or carriage of the Product.
